St. Joseph's Youth Event Forms

 

Before you can attend a St. Joseph's Youth function that is not on parish property (a retreat, a lock-in, etc.), you must turn in a permission slip, signed by a parent/guardian, to the youth office. Even more importantly, though, you need to have a Liability Waiver and Medical Authorization form on file with the youth office. These are effective for one year, starting each July.

The following are the two event forms are PDF format. You will need have Adobe Acrobat Reader installed on your computer to use them. Click the link to download them and then print them out from your desktop printer. If you need a form to be mailed out instead, or you need other help, contact the Webmaster.
